Parisi Academy Acceptance and Deposit

As discussed with you and your parents/guardian, the academic curriculum delivered by Parisi Academy is an advanced eighth-grade and ninth-grade curriculum. The intention of this curriculum is to prepare student-athletes for admissions into a ninth-grade academic program at a public or private high school. Please note that any Parisi Academy recommendations from the staff or faculty for high school admissions will be based on the student-athlete’s success in our program and will be for a ninth-grade academic program only. In addition, we will gladly provide any paperwork and transcripts required for both public and private schools during their application process and at the end of the school year.

Please understand that whether you are spending your initial eighth-grade year or a reclassification eighth-grade enhancement year with Parisi Academy, you are expected to keep a minimum 85% attendance record, INCLUDING EXCUSED AND UNEXCUSED ABSENCES. Otherwise, your transcript will be noted with an incomplete for the grading period(s) in which you fell below 85%.

Additionally, our behavioral, academic and training program standards include high-expectations and we expect you, as a Parisi Academy student-athlete, to rise to the challenge and meet these standards at all times. Our goal is to prepare you for successful ninth-grade entry into high school socially, academically and athletically.

I acknowledge that I, individually, have voluntarily applied to participate in Parisi Academy the Parisi Speed School training program. I acknowledge the risks and the potential risks of athletic training. However, I feel that the possible benefits to me and my child are greater than the risk assumed. I am aware that although The Parisi Academy, Parisi Training Systems, Inc, its subsidiaries, and parent companies, its officers, directors, owners and/or employees make reasonable efforts to make each athlete’s training a safe and productive experience, that there are inherent risks which occur as a result of such physical activity. I acknowledge that an athlete, when training, through no fault of his own, his trainer(s) or the facility may become injured for a variety reasons that are unavoidable. I represent that I am in good health and suffer from no physical impairment, which would limit my use of The Parisi Academy and The Parisi Speed School’s facilities or instruction. I further represent that I carry full and complete medical insurance coverage. I acknowledge that The Parisi Academy and The Parisi Speed School has not and will not render any medical services including medical diagnosis of my physical condition. In consideration of being permitted by The Parisi Academy and The Parisi Speed School to participate its training program and to use its facilities, I hereby, intending to be legally bound for myself, my heirs and assigns, executors or administrators and/or guardian of my son/my daughter/my ward specifically agree that that Parisi Academy and The Parisi Speed School, its officers, employees and agents shall not be liable for any claim, demand, cause of action of any kind whatsoever for, or on account of death, personal injury, property damage or loss of any kind resulting from or related to my use of the facilities or participation in any athletic training, exercise or activity within or outside the club premises, and I agree to hold The Speed Parisi School and Parisi Training Systems, Inc harmless from same. I hereby waive any and all claims for any and all injuries I may suffer under any circumstances, including but not limited to those claims arising from the negligence of The Parisi Academy and The Parisi Speed School, Parisi Training Systems, Inc, its employees, agents, servants, invitees, co-members, contractors, or subcontractors, employees or otherwise. I testify that my child has seen a licensed medical doctor within the last 12 months and has passed a normal routine physical exam and is cleared to participate in physical training activities.

I, understand that my participation and/or involvement in the Parisi Academy is a program designed to supplement my child’s/children’s home school program. I acknowledge that I am fully responsible for satisfying all rules and regulations concerning the home schooling of my child/children and that the Parisi Academy is simply a supplemental program designed to enhance and enrich my student’s education experience and existing home school academic program.

My child is/children are willing participant(s) in the Parisi Academy.

By signing this agreement, I agree to release, indemnify, and hold harmless Parisi Training Systems, Inc., as well as all its employees, agents, representatives, instructors, successors, etc. from all losses, claims, demands, liabilities, causes of action, or expenses, known or unknown, arising out of my participation in the Parisi Academy, including but not limited to any and all fines, penalties, costs, or other repercussions arising out of or related to the failure of my child’s/children’s home school program failing to meet with any governmental requirements.

In additional consideration of being permitted by The Parisi Academy and The Parisi Speed School to participate in its education and training program and to use its facilities, I hereby permit The Parisi Academy and The Parisi Speed School to use my name, image, and likeness for promotional purposes and to also use my child’s name, image, and likeness for promotional purposes. The Parisi Academy and The Parisi Speed School’s promotional mediums include but are not limited to print, radio, video, television and the Internet. I acknowledge that I have read this release and waiver and fully understood its contents.

I understand that participation in the Parisi Academy Program is limited and that once I commit to participation such commitment, including the commitment to pay, is non-cancellable. Space is limited and timing for committing to participating in the program is critical. I further understand that due to the nature of the offering, that Parisi cannot fill a “cancelled space” once the program commences. As such, I acknowledge and agree that upon payment of the initial deposit of $500, all tuition is due and payable in full regardless of the payment plan option we selected. This applies whether or not my student-athlete completes the academic year. All tuition payments are non-refundable.
